Job summary

Easterseals Florida in Gainesville is seeking a compassionate LPN to join our Adult Day Healthcare program. You will provide health monitoring and nursing services for participants in a person-centered setting designed to enhance quality of life.

The role includes admissions coordination, scheduling medical visits, and coordinating care plans with families. We offer health, dental, vision plans, paid time off and a supportive team dedicated to community impact.

Qualifications

• Graduate of an accredited nursing program.
• Current Florida Licensed Practical Nurse License.
• At least one year of experience with individuals with disabilities and/or aging population.
• CPR and/ or First Aid certification is a plus.

Responsibilities

• Assist in coordinating admissions and discharges, and maintain participant records.
• Schedule in-house medical visits and transportation as needed.
• Coordinate quarterly care plan meetings with participants and families.
• Maintain participant records and report trends requiring attention.
