Job summary

Trustbridge Hospice, part of Empath Health, is seeking a compassionate Hospice Licensed Practical Nurse (LPN) to join our Boca area team. The shift is 8:30am–5pm with weekend availability, supporting patients in a hospice setting.

You will assess patient and family needs under RN guidance, develop comfort-focused care plans, and provide overnight support to enhance quality of life. A Florida LPN license and BLS are required.

Qualifications

• Current license to practice as an LPN in Florida (FL).
• Current Basic Life Support Certification (BLS).
• Completion of a 30-hour IV course is a plus per Florida Board of Nursing requirements.

Responsibilities

• Assess patient and family needs under RN and physician guidance.
• Coordinate and implement individualized care plans prioritizing comfort and dignity.
• Use monitoring/teaching skills to support quality of life.
• Provide emotional support to patients and families during overnight care.
• Ensure a compassionate, personalized patient experience in hospice.

Trustbridge Hospice, a part of Empath Health, is seeking a compassionate and dedicated Hospice Licensed Practical Nurse (LPN) to join our team and provide high-quality, patient-centered care to individuals receiving hospice care in the Boca Area.

Schedule: 8:30am - 5pm with weekend availability.

• Work under the direction of the Registered Nurse and primary physician to assess patient and family needs
• Coordinate and provide individualized care plans that prioritize comfort and dignity
• Utilize monitoring, intervention, and teaching skills to enhance quality of life
• Provide emotional support and reassurance to patients and families during overnight care
• Ensure exceptional, personalized service and a positive experience for every patient and family

What You'll Need

• Current license to practice as a Licensed Practical Nurse in the State of Florida.
• Current Basic Life Support Certification.
• Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
