Job summary

Health Advocates Network, Inc. is seeking a Licensed Practical Nurse (LPN) for Sterling, CO to provide direct patient care in a correctional health setting.

The role involves administering medications, monitoring vital signs, and supporting high-quality care under RN/physician supervision. Applicants must hold an active CO LPN license in good standing, have at least 1 year of recent LPN experience, and provide PSV documentation if required.

Qualifications

• Current, valid Colorado LPN license in good standing.
• Minimum of 1 year of LPN experience within the last 3 years.
• PSV copy from licensing board or verification agency if required.
• Current required certifications (e.g., BLS, immunization records, TB test).

Responsibilities

• Provide direct patient care under supervision of RN or physician.
• Administer medications and treatments as prescribed.
• Monitor and record vital signs and report changes.
• Assist with patient assessments and care plans.
• Perform wound care, specimen collection, and basic nursing procedures.
• Document care in EMR.
• Educate patients and families on health maintenance and plans.
• Assist with admissions, discharges, and transfers.

Licensed Practical Nurse Duties & Responsibilities:

• Provide direct patient care under the supervision of a Registered Nurse or physician.
• Administer medications and treatments as prescribed.
• Monitor and record patients' vital signs and report changes in condition.
• Assist with patient assessments and implement individualized care plans.
• Perform wound care, specimen collection, and other basic nursing procedures.
• Document patient care accurately in the electronic medical record (EMR).
• Educate patients and families on health maintenance and treatment plans.
• Assist with admissions, discharges, and transfers.
• Maintain a safe, clean, and therapeutic environment for patients.
• Follow facility policies, infection control protocols, and regulatory standards.

Minimum Qualifications:

• Current, valid, and unrestrictedCO license/certification in good standing, as required for the position.
• Minimum of 1 year/sof recent experience as aLPN within the last 3 years.
• Copy of current, completedPrimary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
• Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test,...)
