Job summary

Health Advocates Network, Inc. is seeking a Licensed Practical Nurse (LPN) in Ridgeville, SC to deliver high-quality patient care in a correctional healthcare setting. The role includes direct patient care under supervision, medication administration, vital signs monitoring, and EMR documentation.

We offer competitive local pay of $40.00/hour (W2) with benefits and opportunities for professional growth. Start date is 10/12/2026; night shift schedule is 6:00 PM–7:00 AM.

Qualifications

• Current, valid SC LPN license in good standing.
• At least 1 year of LPN experience within the last 3 years.
• Primary Source Verification (PSV) from licensing board if required.
• Copies of current required certifications (BLS, N95 fit test, immunizations, TB test).

Responsibilities

• Provide direct patient care under supervision of a RN or physician.
• Administer medications and treatments as prescribed.
• Monitor and record vital signs and report changes.
• Assist with patient assessments and care plans.
• Perform basic nursing procedures and wound care.
• Document care accurately in the EMR.
• Educate patients and families on health maintenance.
• Assist with admissions, discharges and transfers.
• Maintain a safe, clean, therapeutic environment.
• Follow facility policies and infection control protocols.

Licensed Practical Nurse (LPN) Duties & Responsibilities

• Provide direct patient care under the supervision of a Registered Nurse or physician.
• Administer medications and treatments as prescribed.
• Monitor and record patients' vital signs and report changes in condition.
• Assist with patient assessments and implement individualized care plans.
• Perform wound care, specimen collection, and other basic nursing procedures.
• Document patient care accurately in the electronic medical record (EMR).
• Educate patients and families on health maintenance and treatment plans.
• Assist with admissions, discharges, and transfers.
• Maintain a safe, clean, and therapeutic environment for patients.
• Follow facility policies, infection control protocols, and regulatory standards.

Minimum Qualifications

• Current, valid, and unrestricted South Carolina license/certification in good standing, as required for the position.
• Minimum of 1+ year within the last 3 years of recent experience as a LPN
• Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
• Copy of all current required certifications and compliance documents (e.g., . BLS, N95 fit test, immunization records, TB test,..)
