New Horizon Family Health Services, Inc. is seeking a Concierge Licensed Practical Nurse to join our team in Greenville, SC.

This role serves as the first clinical contact for patients, providing symptom assessment, care coordination, and navigation support. You will perform nursing tasks within scope of practice, assist providers with patient flow, document in the EHR, and support coordination of care across multiple sites in Greenville, Greer, Travelers Rest, and Berea.

Competenze

• Graduation from an accredited LPN program is required.
• SC Board of Nursing licensure is required.
• Current BLS/CPR certification.

NOTE: New Horizon Family Health Services operates multiple locations throughout Greenville, Greer, Travelers Rest, and Berea. Candidate placement and worksite assignments will be determined at the discretion of the Executive Team, based on organizational needs and operational priorities.

Job Summary

The Concierge Licensed Practical Nurse assists the Medical Provider in the care and treatment of New Horizon Family Health Services patients. The LPN Concierge shall perform a variety of complex nursing tasks and related duties as required. This position serves as a first point of clinical contact for patients providing symptom assessment, care coordination, and navigation support.

Duties Performed

• Serve as a welcoming clinical touchpoint for patients needing assistance navigating services.
• Address walk-in concerns to assess symptoms and determine appropriate level of care (routine, urgent, or emergent)
• Use clinical judgement, standing orders, and approved protocols to guide care recommendations.
• Escalate complex or urgent cases to appropriate providers.
• Provide education and guidance for symptom management, medication questions, test results, and follow up care.
• Completes weekly assigned chores.
• Coordinate same day and urgent appointments, follow up visits, or referrals in collaboration with front desk, care teams, and referral coordinators.
• Document all patient interactions, assessments, and care decisions accurately in the Electronic Health Record (EHR).
• Is responsible for checking daily patient phone messages.
• Follow up on test results, medication refills, and provider messages.
• Participate in interdisciplinary team huddle meetings and clinical CQI.
• Perform clinical tasks such as vitals, immunizations, wound care, specimen collection, point of care testing, EKG’s, and medication administration per scope of practice.
• Adheres to infection control protocol.
• Will monitor and complete assigned EHR provider staff buckets and task buckets as needed.
• Ensure patient safety during in person interactions.
• Assist providers with rooming patients, examinations, procedures, patient treatments, and other patient care nursing tasks upon request.
• Maintains patient confidentiality.
• Utilizes NHFHS Bloodborne Pathogen protocol upon possible self injury.
• Communicate effectively with patients of diverse cultural and socioeconomic backgrounds, using interpreter services when appropriate.

Qualifications

• EDUCATION:
  • REQUIRED:
    • Graduation from an accredited school of practical nursing and one (1) year of practical nursing experience, preferably in an ambulatory health care setting.
    • Must be licensed as a Practical Nurse by the South Carolina Board of Nursing.
    • Current BLS/CPR certification
• EXPERIENCE:
  • REQUIRED:
    • one (1) year of practical nursing experience, preferably in an ambulatory health care setting.
    • Experience with an electronic health record
• PREFERRED:
  • One (1) year of experience in a community health center or acute care medical practice
