New Horizon Family Health Services, Inc. seeks a Clinical Support Specialist to assist Care Teams with referrals and patient flow under the supervision of the Program Director/Mallard Site Manager.

The role focuses on processing patient information, coordinating referrals, and maintaining strong communication across the health care team. Responsibilities include scheduling, obtaining pre-authorizations, and ensuring timely delivery of information through the EHR while upholding patient

• High school diploma or GED; CNA/MA/PCT or 1 year related experience required.
• Electronic Health Records experience required.

NOTE: New Horizon Family Health Services operates multiple locations throughout Greenville, Greer, Travelers Rest, and Berea. Candidate placement and worksite assignments will be determined at the discretion of the Executive Team, based on organizational needs and operational priorities.

Job Summary

The Clinical Support Specialist works under the supervision of the Program Director/ Mallard Site Manager to provide referral and patient flow support to NHFHS Care Teams.

Duties Performed

• Demonstrates a working knowledge of the EHR, clerical and referral processes.
• Receives and processes patient-related information from multiple sources, ensuring timely and accurate delivery to the appropriate departments or individuals.
• Helps support the achievement of provider productivity goals.
• Establishes and maintains effective working relationships with all members of the health care team and demonstrates appropriate communication skills.
• Works to process provider referral orders in a timely fashion and forwards necessary EHR information as needed and required for each individual order.
• Schedules In-House referrals as needed and follows established processes and guidelines.
• Schedules External referrals for various procedures including, but not limited to, radiological procedures and consultations with specialists as ordered by medical providers and directs patients accordingly.
• Obtains necessary referral pre-authorizations.
• Records for referral orders are forwarded via EHR Fax.
• Communicates all “stat” reports/issues immediately to appropriate team members (MD, FNP, RN / LPN, MA, PM, or CN).
• If unable to complete a referral, reports immediately to the referring Provider.
• Understands the purpose of and has a working knowledge of Patient Centered Medical Home concepts.
• Tracking: works with Task Management and clinical staff, as needed, to track outcomes for referrals, hospitalizations, pap smears, vouchers and other required information supporting Patient Centered Medical Home guidelines.
• Communicates clerical supply needs to the immediate supervisor in a timely manner.
• Secures necessary referral pre-authorizations in accordance with payer guidelines and organizational policies.
• Contributes constructive ideas for a more efficient referral process.
• Attends staff development programs as required.
• Attends monthly staff meetings / developmental programs and contributes constructive ideas for more efficient referral processes.
• Maintains confidentiality of patient information at all times.

Qualifications

• EDUCATION :
  • REQUIRED :
    • High school diploma or GED equivalent.
    • CNA. MA, PCT or minimum of 1 year of related work experience.
  • PREFERRED:
    • 1-2 years higher education in related field or social work fields.
• EXPERIENCE :
  • REQUIRED :
    • Electronic Health Records Experience
  • PREFERRED :
    • One year experience within a community healthcare setting
