Job summary

Active Day is seeking a Licensed Practical Nurse to provide nursing care to members under physician orders and emergent needs in our Kentucky center. You will deliver instruction and assistance to members, staff and families while ensuring all work aligns with company policies, safety, and regulatory standards.

Qualifications

• LPN license and CPR certification are required.
• Must be at least 21 years old.
• Experience in healthcare setting preferred (1+ year FTE).
• Experience with seniors or adults with special needs preferred.
• Strong communication and interpersonal skills.
• Ability to multitask and manage time effectively.
• Flexible schedule availability.
• Must be able to lift up to 50 pounds and assist members physically.
• Background check passed and any state regulatory requirements.

Responsibilities

• Provide nursing care per plan of care, orders, and emergent needs.
• Monitor and document vital signs and responses to treatments.
• Assist with daily living activities: hygiene, meals, ambulation, social activities.
• Provide health education to staff, members, and families.
• Provide emergency care as required.
• May administer medications with proper certification and supervision.
• Communicate professionally with members, families, colleagues.
• Participate in staff meetings and cross-functional collaboration.
• Comply with policies, safety, and regulatory standards.
• Other duties as assigned.
