Job summary

Active Day is seeking a Licensed Practical Nurse to provide nursing care to members per physician orders and emergent needs in a on-site setting. You will help with daily activities, monitor vital signs, and educate staff and families.

The role requires an LPN license, CPR, and willingness to meet background checks and state requirements. Strong communication and the ability to multitask are essential for success.

Qualifications

• LPN license, CPR certification, and malpractice insurance if applicable.
• At least 21 years of age.
• At least one year full-time equivalent experience in a health care setting preferred.
• Prior experience with seniors or adults with special needs preferred.
• Strong communication and engaging interpersonal skills.
• Able to multi-task and manage time effectively.
• Able to work a flexible schedule.
• Able to bend, lift 50 pounds; stand for extended periods.
• Background check completed; comply with state requirements.

Responsibilities

• Provides nursing care to members according to care plan and physician orders.
• Observes and documents vital signs and responses to treatments; report abnormalities.
• Assists with daily activities including hygiene, meals, ambulation, and social activities.
• Provides health education and instruction to staff, members and families.
• Provides emergency care as required.
• May administer medications with appropriate certification and supervision by RN.
• Communicate professionally with members, families, coworkers, and community.
• Participate in staff meetings and education; collaborate with cross-functional teams.
• Comply with policies, safety and regulatory standards.
• Other duties as assigned.
