Job summary

Topaz Fiscal Services in Kentucky seeks a licensed practical nurse (LVN/LPN) to provide quality nursing care for residents on the assigned unit. NEW graduates are welcome to apply and will support routine nursing tasks and care delivery.

You will administer prescribed medications, explain treatments, monitor vital signs, and document outcomes while following the care plan and participating in care conferences with the DON/ Administrator.

Qualifications

• Current State of LVN/LPN with a minimum of 2 years related experience; or equivalent combination of education and experience. Good standing in the state registry is required.
• Provides nursing care to residents and follows approved procedures and care plans.
• Administers prescribed medications and treatments in accordance with nursing techniques.

Responsibilities

• Responsible for total nursing care of residents in the assigned unit.
• Explains procedures and treatments and care plans to residents.
• Charts progress notes reflecting care and resident response.
• Takes vital signs and monitors for deviations from normal.
• Responds to emergencies and follows policy and procedures.
• Coordinates care plans and collaborates with therapists and dietary staff.
• Ensures documentation reflects the care plan and is updated daily.
