Job summary

Active Day in Lexington, KY is seeking a Licensed Practical Nurse to provide nursing care to members according to physician orders and emergent needs. You will educate and assist members, staff, and families while complying with company policies, safety standards, and regulatory laws.

The position involves monitoring vital signs, assisting with daily activities, health education, and potential medication administration under RN supervision. Must meet physical demands and pass background checks.

Qualifications

• LPN license and CPR certification required.
• Must be at least 21 years of age.
• One year full-time equivalent healthcare experience preferred.
• Prior experience with seniors or adults with special needs preferred.
• Strong communication and interpersonal skills.
• Ability to multi-task and manage time effectively.
• Flexible schedule required.
• Physical ability to bend, lift up to 50 pounds, and stand for extended periods.
• Background checks and any state regulatory requirements as applicable.

Responsibilities

• Provides nursing care to members according to individual plan of care, physician's orders and emergency needs.
• Observes, monitors and documents members' vital signs and responses to treatments and medications, reporting abnormalities to Director of Nursing.
• Assists members with daily functional activities, including hygiene, meals, ambulation, arrivals and departures, and social activities.
• Provides health education and instruction to staff, members and family members.
• Provides emergency care as required.
• May be required to administer medications with appropriate certification and supervision by the Registered Nurse.
• Communicate in a positive, courteous and professional manner with members and their families, co-workers, referral agencies, and the community at large.
• Participate in staff meetings and in-service educational programs as requested; actively collaborate with cross-functional team, providing feedback and recommendations for enhancing the member experience and achieving the center's overall goals and objectives.
• Comply with company policies and procedures, safety and regulatory laws and standards.
• Other duties and responsibilities as changed or assigned at any time.
