Job summary

You will collaborate with CNAs, RNs, providers, and therapists to ensure comfort, health, and independence for residents, while maintaining accurate documentation and timely communication of changes in condition.

• Active and valid LPN license in good standing.
• Able to work on-site.

Responsibilities

• Provide direct nursing care per resident care plans and physician/provider orders.
• Administer medications and treatments safely and accurately.
• Monitor residents for changes in condition and report promptly.
• Complete nursing documentation accurately and in a timely manner.
• Work closely with CNAs and other members of the care team to coordinate resident care.
• Communicate with residents, families, providers, and other healthcare professionals.
• Respond to resident needs and help create a safe, comfortable, and respectful environment.
• Build meaningful relationships with the residents you serve through compassionate and attentive care.
