Transitions Healthcare North Huntingdon is hiring Licensed Practical Nurses (LPNs) to join our compassionate care team. You will provide high-quality nursing care, collaborate with RNs and physicians, and support residents and their families in a resident-centered environment.

Qualifications

• Must be a graduate of an accredited Licensed Practical Nursing (LPN) program.
• Current LPN license in good standing with the Commonwealth of Pennsylvania.
• Strong clinical, communication, organizational, and documentation skills.
• Prior long-term care, rehabilitation, or skilled nursing experience preferred, but not required.

Responsibilities

• Administer medications and treatments as prescribed while monitoring residents.
• Obtain and record vital signs and clinical observations.
• Assess, monitor, and document resident conditions and report changes promptly to RNs/physicians.
• Maintain complete, accurate, and timely clinical documentation.
• Develop and implement individualized resident care plans.
• Communicate effectively with residents, families, physicians, and the care team.
• Provide compassionate, resident-centered nursing care.
• Supervise CNAs to ensure high-quality care.
• Respond to emergencies and changes in resident condition.
• Follow facility policies, state and federal regulations, and infection prevention standards.
