Emerald Nursing & Rehab Mercy in Nebraska is seeking a Licensed Practical Nurse to join our team on day/night shifts. You will provide direct nursing care, administer medications, monitor residents, and collaborate with RNs, CNAs, and therapists to support resident well-being.

The role emphasizes compassionate care, accurate documentation, infection control, and clear communication with residents' families.

Qualifications

• Valid LPN/LVN license in Nebraska.
• Graduation from an accredited LPN/LVN program.
• Previous experience in a skilled nursing facility or similar healthcare setting preferred.
• Current CPR/BLS certification required.
• Excellent verbal and written communication skills.
• Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families.

Responsibilities

• Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
• Monitor residents' vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
• Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident's needs.
• Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.
• Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
• Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
• Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.
