Qualifications

• Current LPN license in Pennsylvania.
• Ability to lift, transfer, and reposition patients; stand/walk for long periods.
• Strong communication and interpersonal skills in a fast-paced environment.

Responsibilities

• Participate in the delivery of quality patient care under the supervision of the Registered Nurse.
• Administer medications, treatments, and therapies as ordered by the healthcare provider.
• Conduct vital signs, perform basic wound care, and assist with mobility and ambulation.
• Develop and implement individualized care plans with the interdisciplinary team.
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
• Assist with daily living activities such as bathing, dressing, and grooming as needed.
• Participate in quality improvement initiatives to ensure optimal patient outcomes.
