Job summary

Ellen Memorial Rehabilitation and Healthcare Center LLC in Honesdale, PA is seeking a compassionate LPN Licensed Practical Nurse to provide high-quality patient care to our residents, including medication administration, vital sign monitoring, wound care, ADLs, and collaborative care planning.

You will work with an interdisciplinary team, document care accurately, participate in ongoing education, and contribute to positive patient outcomes in a supportive, growth-oriented setting.

Qualifications

• Provides direct patient care including medication administration and treatments.
• Monitors vital signs and reports changes to the nursing team.
• Performs wound care and dressing changes as needed.
• Assists residents with activities of daily living (ADLs) and personal care.

Responsibilities

• Provide direct patient care, including administering medications, performing treatments, and providing assistance with ADLs
• Participate in the development and implementation of individualized care plans
• Monitor and report changes in patient condition to the nursing team
• Maintain accurate and detailed records of patient care and treatment
• Collaborate with the interdisciplinary team to ensure quality patient care
• Participate in continuing education and professional development opportunities to stay current with industry best practices
