Our Licensed Practical Nurses support the medical team in residences, administer medications, and assist in coordinating care under the supervision of a Registered Nurse.

Qualifications

• Must be a graduate of an accredited nursing program.
• At least one year of experience as an LPN in community-based health services.
• Current NY LPN license and registration.
• Ability to read, write, speak and understand English.

Responsibilities

• Assist Registered Nurse in coordinating health care services for assigned residences.
• Keeps progress notes and summarizes for reviews.
• Provides health care and hygiene instructions to individuals and families.
• Manages infection control and reports findings to RN or physician.
• Administers medications and oversees storage.
• Attends appointments and team meetings; on-call as needed.
