LifeStream Behavioral Center, Inc. is seeking a Licensed Practical Nurse (LPN) to provide high-quality nursing care within a behavioral health setting in Leesburg, FL.

The LPN will support the healthcare team, contribute to patient care plans, and ensure safe, compassionate care for patients. You will perform nursing assessments, document accurately, administer medications as directed, and maintain compliance with infection control and documentation standards.

Qualifications

Responsibilities

• Assist in developing and implementing patient care plans with RNs and healthcare professionals.
• Perform nursing assessments, documentation, and administer psychopharmacology as directed.
• Maintain timely documentation of Medication Profiles and Clinic Review forms.
• Record state outcome measures and collect QA measures when needed.
• Participate in team meetings and improve care processes and protocols.

Job description

Job Purpose:- The Licensed Practical Nurse (LPN) is responsible for providing high-quality nursing care to patients within the behavioral health setting. This role is pivotal in ensuring the well-being and safety of patients, by supporting the healthcare team in delivering comprehensive and compassionate care.Key Responsibilities:- Assist in the development and implementation of patient care plans, collaborating with registered nurses and other healthcare professionals.- Satisfactory nursing assessment and documentation, psychopharmacology, and provide effective patient care- Will maintain accurate and timely documentation in the medical record of the Medication Profile, Med Clinic Review form, etc..- Responsible for recording state outcome measures for clients without an assigned or primary clinician and collect QI measures when needed- Participate in team meetings and contribute to the continuous improvement of care processes and protocols within the center.- Ensure a safe and clean environment for patients by adhering to infection control policies and procedures.- Respond promptly to emergency situations, providing appropriate interventions and notifying the necessary medical staff.Required Education:- Completion of an accredited Licensed Practical Nurse (LPN) program.- Current and valid LPN license in the state of practice.Required Experience:- 0 to 3 years of experience working as a Licensed Practical Nurse, preferably in a behavioral health setting.- Experience with patient care in a clinical environment, including administering medication and monitoring patient health.Required Skills and Abilities:- Strong understanding of nursing practices and principles, particularly in a behavioral health context.- Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare team members.- Ability to work well under pressure and handle challenging situations with professionalism and empathy.- Proficient in maintaining accurate and detailed patient records.- Demonstrated ability to follow healthcare protocols and procedures.- Strong organizational skills and attention to detail.- Ability to work collaboratively as part of a multidisciplinary team.LifeStream BenefitsHealth/Dental/Vision InsuranceShort Term DisabilityPension Plan403(b)Paid Time Off (Over 4 weeks your 1st year!)Flexible Work SchedulesTuition Reimbursement ProgramFree Telehealth ServicesHRSA Loan Forgiveness ProgramAnd More!Important NoticeAs part of our hiring process and in compliance with Section 435.04, Florida Statutes, certain positions require a Level 2 background screening.
