Job summary

LIFESTREAM BEHAVIORAL CENTER, Clermont, FL, is seeking a Licensed Practical Nurse (LPN) to provide high-quality nursing care within a behavioral health setting. You will work under supervision of RNs and physicians during Day shift (Shift 1) to ensure compassionate patient care.

The ideal candidate has completed an accredited LPN program and up to 3 years of practical nursing experience in a clinical or behavioral health environment.

Qualifications

• Completion of an accredited LPN program.
• 0–3 years of practical nursing experience, preferably in a behavioral health.
• Experience in patient care and monitoring in a clinical environment.
• Strong understanding of nursing procedures and protocols.
• Excellent communication with patients, families, and healthcare team.

Responsibilities

• Administer medications and treatments as prescribed by physicians and in accordance with nursing standards and protocols.
• Monitor vital signs, symptoms, and medical histories and report significant changes to supervising nurse or physician.
• Assist in developing and implementing patient care plans tailored to individual needs.
• Provide emotional support and education to patients and families about treatment plans and procedures.
• Maintain accurate patient records, ensuring confidentiality and compliance with healthcare regulations.
• Collaborate with a multidisciplinary team to coordinate patient care.
• Participate in ongoing training and professional development to enhance nursing skills.
• Ensure a safe and therapeutic environment by following safety protocols.
• Assist with admission, transfer, and discharge processes to ensure continuity of care.

Job Location: South Lake Residential - Clermont, FL, Position Type: Full Time, Salary Range: $27.00, Job Shift: Day, Job Purpose: The Licensed Practical Nurse (LPN) at LIFESTREAM BEHAVIORAL CENTER is responsible for providing high-quality nursing care to patients within the behavioral health setting. This role involves working under the supervision of registered nurses and physicians to ensure the delivery of compassionate and effective patient care during Shift 1.

Key Responsibilities

• Administer medications and treatments as prescribed by physicians and in accordance with nursing standards and protocols.


• Monitor and record patient vital signs, symptoms, and medical histories, and report any significant changes to the supervising nurse or physician.


• Assist in the development and implementation of patient care plans, ensuring they are tailored to meet the individual needs of each patient.


• Provide emotional support and education to patients and their families, helping them understand treatment plans and healthcare procedures.


• Maintain accurate and detailed patient records, ensuring confidentiality and compliance with healthcare regulations.


• Collaborate with a multidisciplinary team to coordinate patient care and ensure a holistic approach to treatment.

Required Education

• Completion of an accredited Licensed Practical Nurse (LPN) program.

Required Experience

• 0 to 3 years of practical nursing experience, preferably in a behavioral health or similar healthcare setting.


• Experience in providing patient care and monitoring in a clinical environment.

Required Skills and Abilities

• Strong understanding of nursing care procedures and protocols.


• Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare team members.


• Ability to work effectively in a team-oriented environment while maintaining a high level of professionalism.


• Proficiency in using healthcare-related software and electronic health records.


• Strong organizational skills and attention to detail to ensure accurate documentation and patient care.
