Job summary

Harbor Healthcare System in Independence, MO is seeking a full-time Licensed Practical Nurse (LPN) Clinical Liaison/Marketing to determine hospice eligibility, communicate with families, and cultivate trusted relationships with local providers to advance compassionate care.

The role collaborates with the facility’s marketing and admissions teams, schedules community events, speaks at engagements, travels as needed, reviews medical records, explains care options, and supports smooth transitions

Qualifications

• Licensed Practical Nurse (LPN) licensed in Kansas (required).
• Experience in Nursing Home, Hospice or Home Health Marketing preferred.
• Highly organized and detail oriented.
• Effective written and verbal communication.
• Ability to travel with reliable transportation.
• Clean background and drug screen.

Responsibilities

• Determine hospice eligibility for patients.
• Communicate with families of patients.
• Interact with hospital staff from multiple departments.
• Assess patients on admission and review medical records.
• Explain care options to patients and families.
• Develop community relationships for the facility.
• Schedule speaking engagements at community events.
• Collaborate with marketing and sales to raise awareness of services.
