Elara Caring is seeking a Licensed Practical Nurse Clinical Coordinator to join our team and advance home health services. In this role, you will support clinical teams, coordinate patient care, and manage documentation and administrative tasks across the assigned branch.

Responsibilities include reviewing HCHB notes, obtaining physician orders, maintaining the At-Risk registry, assisting patients with scheduling and authorizations, and conducting field visits as needed.

Qualifications

• Current, unrestricted LPN license in the state of employment.
• Graduate of an accredited school of nursing.
• Minimum of 1 year of nursing experience in a clinical setting.
• Ability to lift, push, pull, and move up to 50 pounds regularly.
• Ability to work in an office setting and travel within the branch area.
• Valid driver's license or reliable transportation for travel between locations.
• Bilingual English/Spanish preferred.

• Monitor and maintain the At-Risk patient registry to support quality patient care.
• Assist patients with scheduling, authorizations, and addressing questions or concerns.
• Complete patient chart audits and maintain accurate clinical documentation.
• Verify required Medicare notices are provided to patients and maintain patient confidentiality.
• Collaborate with clinical staff and case managers to ensure coordinated patient care.
• Provide direct patient care through field visits as needed.

What is Required?

• Current, unrestricted Licensed Practical Nurse (LPN) license in the state of employment.
• Graduate of an accredited school of nursing.
• Minimum of 1 year of nursing experience in a clinical setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Ability to work in an office setting and travel within the assigned branch coverage area as needed.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between assigned work locations.
• Bilingual in English and Spanish is preferred.
