Job summary

Elara Caring is seeking a Licensed Practical Nurse Clinical Coordinator to advance patient care by coordinating clinical teams, reviewing Homecare Homebase (HCHB) notes, and entering physician orders for approval.

The role requires an LPN license, at least 1 year of clinical nursing experience, ability to lift up to 50 pounds, and willingness to travel within the branch coverage area. Bilingual English/Spanish is preferred.

Qualifications

• Current, unrestricted LPN license in the state of employment.
• Graduate of an accredited school of nursing.
• Minimum of 1 year of nursing experience in a clinical setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients.
• Ability to work in an office setting and travel within the assigned branch coverage area as needed.

Responsibilities

• Coordinate patient care with clinical teams and maintain efficient workflows.
• Review and process Homecare Homebase (HCHB) coordination notes and administrative tasks accurately and on time.
• Obtain and enter physician orders for Clinical Team Manager review and approval.
• Monitor the At-Risk patient registry to support quality patient care.
• Assist patients with scheduling, authorizations, and addressing questions or concerns.
• Complete patient chart audits and maintain accurate clinical documentation.
• Verify required Medicare notices are provided to patients and maintain patient confidentiality.
• Collaborate with clinical staff and case managers to ensure coordinated patient care.
• Provide direct patient care through field visits as needed.

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between assigned work locations.
• Bilingual in English and Spanish is preferred.
