Elara Caring is seeking a Licensed Practical Nurse Clinical Coordinator to support clinical teams, coordinate patient care, and handle physician orders. The role involves field visits, maintenance of the At-Risk registry, and ensuring accurate documentation across workflows.

Requirements include an active LPN license, nursing degree, and at least 1 year of clinical experience. Travel between locations and bilingual English/Spanish is preferred, with comprehensive benefits and growth

• Minimum of 1 year of nursing experience in a clinical setting.
• Ability to travel between assigned work locations.

Responsabilités

• Coordinate patient care with clinical teams and maintain efficient workflows.
• Review and process Homecare Homebase (HCHB) coordination notes and tasks.
• Enter physician orders for Team Manager review and approval.
• Monitor the At-Risk patient registry to support quality care.
• Assist patients with scheduling, authorizations, and questions.
• Complete patient chart audits and maintain documentation.
• Ensure Medicare notices are provided and patient confidentiality is maintained.
• Collaborate with staff and case managers to coordinate care.
• Provide direct patient care through field visits as needed.

Job Description

As a Licensed Practical Nurse Clinical Coordinator, you'll contribute to our success in the following ways:

• Support clinical teams in coordinating patient care and maintaining efficient workflows.
• Review and process Homecare Homebase (HCHB) coordination notes and administrative tasks accurately and on time.
• Obtain and enter physician orders for Clinical Team Manager review and approval.
• Monitor and maintain the At-Risk patient registry to support quality patient care.
• Assist patients with scheduling, authorizations, and addressing questions or concerns.
• Complete patient chart audits and maintain accurate clinical documentation.
• Verify required Medicare notices are provided to patients and maintain patient confidentiality.
• Collaborate with clinical staff and case managers to ensure coordinated patient care.
• Provide direct patient care through field visits as needed.

What is Required?

• Current, unrestricted Licensed Practical Nurse (LPN) license in the state of employment.
• Graduate of an accredited school of nursing.
• Minimum of 1 year of nursing experience in a clinical setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Ability to work in an office setting and travel within the assigned branch coverage area as needed.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between assigned work locations.
• Bilingual in English and Spanish is preferred.
