Qualifications

Responsibilities

• Enter patient status changes into the EMR promptly.
• Maintain up-to-date team census and disaster preparedness spreadsheets.
• Prepare daily status reports and enter care plan templates.
• Support IDG meetings by distributing agendas and ensuring attendance.
• Notify team members and external providers of patient status changes.
• Print and file patient care plans and manage weekly reports.
• Assist with special tasks such as census reporting and requisitions.
• Uphold patient rights and provide compassionate support to patients and families.
