Job summary

VNA Care is seeking an LPN Clinical Admissions Coordinator to join our Worcester team. The role focuses on reviewing and coordinating clinical information prior to patient admission to create the best patient pathway and to ensure a high level of customer service.

You will communicate with branches and teams to coordinate new patients and maintain positive relationships with internal and external customers. Compensation is based on experience, with a typical range of $28.00–$32.00 per hour.

Qualifications

• Current LPN license required.
• Minimum 2 years related experience including medical/surgical and community health and/or experience with a managed care organization.
• Previous intake, liaison or discharge planning experience preferred.
• Knowledge of home care standards and practices required.
• Knowledge of medical terminology and treatment modalities of patient diagnoses required.

VNA Care is seeking an LPN Clinical Admission Coordinator to join our Worcester team.

Position Summary:

Reviews and coordinates all needed clinical information prior to patient admission to ensure the perfect pathway for new patients and to ensure an exceptional customer service experience for the patient. Communicates with branches and teams regarding new patients and maintains positive, professional relationships with all internal and external customers.

Compensation based on experience: $28.00 - $32.00

Schedule:

On-Site: Monday, Wednesday 8:00AM-4:30PM; VNA Care 67 Millbrook Street Worcester, MA 01606

Remote: Tuesday, Thursday, Friday 8:00AM-4:30PM

Weekend and Holiday rotation

Current LPN License required.

Experience:
