Responsibilities extend to health screenings, preventive counseling, medication reviews, and timely follow-up. Strong communication and adherence to CNA/LPN standards are essential to ensure safe, patient-centered care.

Qualifications

• High School diploma, GED or equivalent is required.
• Must complete an accredited LPN program.
• CPR certification required.
• Licensed Practical Nurse license required.

Responsibilities

• Triage calls and assess patient symptoms, prioritizing appointments.
• Obtain patient vital signs and prepare for physician examination.
• Review and update beneficiary medical and family history.
• Develop personalized patient prevention plans.
• Complete health risk, social determinants of health, and functional assessments.
• Review and update medications, specialists, and equipment lists.
• Support preventive measures such as health screenings and HEIDS measures.
• Update labs, vaccinations orders and referrals as needed.
• Provide health education referrals and counseled prevention services.
• Assist physicians with in-office procedures and follow-up calls.
