Job summary

Signature HealthCARE of Roanoke Rapids is seeking a licensed practical nurse/licensed vocational nurse to provide direct patient care, including medication administration and wound care, while supporting residents with daily activities. Qualifications

• Current, active LPN/LVN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• CPR/BLS certification preferred.
• Familiarity with electronic health records (EHR) and Microsoft Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the shift in accordance with policies.
• Monitor residents’ status; notify physician and responsible party of changes.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education; perform other duties as assigned.
