Job summary

Signature HealthCARE of Roanoke Rapids is seeking a compassionate LPN/LVN to provide direct nursing care, assist with ADLs, and supervise nursing aides. You will coordinate care plans, document patient status, and ensure safety and quality for residents in a 108-bed facility.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory/charge nurse role preferred.
• Strong observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Familiarity with EHR systems and MS Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs to ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides on shift.
• Monitor residents' status and notify physician of changes.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education.
• Perform other duties as assigned by the Director of Nursing.
