Job summary

Greene Point Health & Rehab is seeking a Licensed Practical Nurse to deliver compassionate, patient-centered nursing care in a supportive team environment. You will collaborate with RNs, CNAs and the interdisciplinary team to administer medications, monitor conditions and maintain care plans.

Applicants must hold an active Georgia LPN license or be eligible, with strong clinical judgment and communication skills. Long-term care experience is preferred but not required.

Qualifications

• Active Licensed Practical Nurse license (Georgia) or eligible.
• Experience delivering compassionate, high-quality nursing care.
• Strong communication and teamwork abilities.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with clinical policies and regulatory standards.
• Communicate effectively with residents, families, and team members.

Your Impact as an LPN

As a Licensed Practical Nurse, you play a critical role in delivering high-quality clinical care and ensuring the well-being of our patients. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care.
