Job summary

Four County Health and Rehabilitation is seeking an LPN to provide compassionate, high-quality nursing care in a non-profit setting. This PRN role covers day/evening/night/weekend shifts and includes supervising CNAs, documenting care, and collaborating with the care team to maintain optimal resident health.

Responsibilities

• Administer medications and treatments as prescribed
• Monitor residents’ conditions and report changes to the care team
• Provide direct nursing care in accordance with care plans
• Supervise and support CNAs in delivering quality care; maintain accurate and timely documentation
• Ensure compliance with all clinical policies and regulatory standards
• Communicate effectively with residents, families, and team members

What You Bring

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible)

• Prior long-term care experience preferred, but not required
