Four County Health & Rehabilitation in Georgia is seeking a Licensed Practical Nurse (LPN) for an evening, full-time role. You will deliver high-quality clinical care, collaborate with RNs and CNAs, and support care plans to promote resident well‑being.

The position emphasizes compassionate patient care, strong clinical judgment, and effective teamwork, with competitive pay and comprehensive benefits. Evening shifts include additional pay differentials and weekly pay.

Qualifications

• Active LPN license in Georgia or eligibility.

• Willingness to work evenings, nights and weekends.

Responsabilités

• Administer medications and treatments as prescribed
• Monitor residents' conditions and report changes to the care team
• Provide direct nursing care in accordance with care plans
• Supervise and support CNAs in delivering quality care
• Maintain accurate and timely documentation
• Ensure compliance with all clinical policies and regulatory standards
• Communicate effectively with residents, families, and team members

Starting Pay: $28.49 to $31.40/hour

Your Impact as an LPN

As a Licensed Practical Nurse, you play a critical role in delivering high-quality clinical care and ensuring the well-being of our patients. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care.

What You Bring

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible)

• Prior long-term care experience preferred, but not required
