Job summary

Signature HealthCARE is seeking a dedicated LPN/LVN to join our Lexington facilities. You will provide direct nursing care, manage medications, and assist residents with daily activities, ensuring dignity and safety.

This role includes supervising nursing staff and maintaining accurate clinical documentation. Required is an active LPN/LVN license with 1+ year of clinical nursing experience; supervisory experience is a plus.

Qualifications

Current, active LPN/LVN license in the state of practice.
CPR/BLS certification preferred.
Minimum one year of clinical nursing experience; long-term care experience preferred.
Experience in a supervisory or charge nurse role preferred.
Strong clinical observation, medication administration, and documentation skills.
Knowledge of Federal and State long-term care regulations.
Effective verbal and written communication skills.
Familiarity with electronic health records (EHR) systems.
Proficiency with Microsoft Office applications.

Responsibilities

Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
Assist residents with activities of daily living and ensure comfort, dignity, and safety.
Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
Monitor and observe residents' physical and emotional status; notify the attending physician and responsible party of any change in condition.
Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
Complete accurate and timely clinical documentation per facility policy and procedure.
Coordinate resident admissions, transfers, and discharges according to established procedures.
Participate in quality assurance activities, staff education, and regulatory surveys as needed.
Perform other duties as assigned by the Director of Nursing or designee.
