Apple Rehab West Haven is seeking a LPN Charge Nurse on a per diem basis to join a compassionate, family-oriented team in West Haven, CT. The role involves planning, coordinating, and supervising nursing services for residents, with on-site shifts and rotating weekend duties.

Starting pay ranges from $30 to $37 per hour, dependent on licensure and experience. Per diem staff will cover two weekend shifts monthly and participate in the on-call rotation.

Qualifications

• Valid state nursing license in good standing (or in process to become RN).
• In-depth knowledge of nursing practices and long-term care regulations.
• Strong attention to detail, excellent organizational skills, multitasking ability.
• Collaborative team player with long-term care experience preferred.

Responsibilities

• Plans and provides professional care for residents in line with needs and changes in medical landscape.
• Maintains professionalism in interactions with residents, physicians, families, peers, and support staff.
• Documents crucial resident care information accurately and comprehensively.
• Administers prescribed medications and ensures accurate record-keeping per policies.
