Job summary

Four County Health & Rehab in Georgia is seeking a licensed practical nurse to join our compassionate, mission-driven team. You’ll administer medications, monitor residents, and provide direct nursing care in collaboration with RNs and CNAs.

This full-time evening position offers shift differentials, weekly pay, and a comprehensive benefits package including health, dental, vision, life and disability insurance, a 401(k) match, paid time off, and holiday pay.

Qualifications

• Active LPN license GA or eligibility is required.
• Passion for delivering compassionate, high-quality care.
• Strong clinical judgment and teamwork abilities.
• Excellent communication and reliability.
• Prior long-term care experience preferred but not required.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with clinical policies and regulatory standards.
• Communicate effectively with residents, families, and team members.

Your Impact as an LPN

As a Licensed Practical Nurse, you play a critical role in delivering high-quality clinical care and ensuring the well-being of our patients. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care.
