Job summary

Townsend Park Health & Rehab is seeking an LPN to deliver high‑quality, patient‑centered care in a supportive, mission‑driven setting. You’ll collaborate with RNs and CNAs to implement care plans and maintain documentation across shifts.

Qualifications

• Active Licensed Practical Nurse (LPN) license (State of Georgia or eligible).
• Commitment to compassionate, high-quality care.
• Strong clinical judgment and teamwork with RNs and CNAs.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care per care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with clinical policies and regulatory standards.
• Communicate effectively with residents, families, and team members.

Your Impact as an LPN

As a Licensed Practical Nurse, you play a critical role in delivering high-quality clinical care and ensuring the well-being of our patients. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person-centered care.

What You Bring
