Qualifications

• Current full, unrestricted nursing license in Alaska or other U.S. state required.
• Graduate of an accredited school of nursing (Bachelor’s degree preferred) or Licensed Practical Nurse (LPN).
• Bachelor’s degree in social work, social service, health administration, or related field.
• Basic Life Support (BLS) certification required.
• High school diploma or equivalent required.

Responsibilities

• Coordinate and deliver care that is safe, timely, effective, efficient, equitable, and patient-centered.
• Assess self-management skills of patients and caregivers and encourage wellness and autonomy through support, training, and coordination of community and/or referred services.
• Communicate regularly with patients and caregivers regarding Plan of Care (POC).
• Coordinate within and outside of the SEARHC health system to implement a consistent, effective care system.
• Establish patient caseload in target communities and track progress and outcomes.
• Maintain knowledge of existing community services and collaborate to close gaps.
• Perform other duties as assigned, including special projects.
