Job summary

Peregrine Senior Living in Albany, NY, is seeking a compassionate LPN Case Manager Assistant to support the Memory Care department with administrative, documentation, and case management tasks. The role collaborates with the Memory Care Director and Case Manager to ensure timely follow-up and accurate resident records.

The ideal candidate holds an active LPN license, has prior senior living experience, and demonstrates strong organizational and communication skills while safeguarding resident

Qualifications

• Active LPN license in good standing.
• Prior experience in senior living, assisted living, memory care, healthcare, or a related setting.
• Strong organizational and administrative skills with attention to detail.
• Ability to communicate effectively and compassionately with residents, families, and care team.
• Maintain confidentiality and handle sensitive resident information appropriately.
• Capable of independent work while collaborating with Memory Care Director.

Responsibilities

• Provide administrative and case management support to the Memory Care department.
• Assist with assigned case management tasks and follow-up activities for residents.
• Maintain accurate, timely, and organized resident documentation and records.
• Assist with gathering and organizing information needed for resident assessments and care planning.
• Help coordinate communication and follow-up with residents, families, and care team as directed.
• Support the Case Manager and Memory Care Director in monitoring resident needs and ensuring follow-up.
• Maintain documentation and records per company policies and regulations.
• Collaborate with team to promote resident well-being, dignity, rights, and quality of life.
