Job summary

Trilogy Health Services, LLC is seeking an LPN to provide direct clinical care and supervise nursing assistants for residents on the unit. You’ll observe residents’ physical and emotional status and communicate changes to the charge nurse and physician.

The role emphasizes holistic care, medication inventory oversight, and completion of medical documentation in a supportive long-term care setting in Bardstown, KY. This position requires current LPN licensure and CPR certification.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s status and notify nursing leadership of changes.
• Identify problems and emergency situations and initiate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within LPN scope.
• Monitor medications, supplies and equipment to ensure availability.
• Complete medical forms, reports and evaluations as needed.
• Report occupational exposures following facility policies.
