Job summary

The Wright Center for Community Health is hiring a Licensed Practical Nurse (LPN) Care Coordinator to join a fast-paced outpatient team. You will coordinate patient care, manage follow-ups, and support clinicians with medication preparation and monitoring devices.

Ideal candidates have an active LPN license, BLS certification, and experience with electronic health records such as Medent. Excellent communication and teamwork are essential for success in this role.

Qualifications

• Active LPN license in good standing.
• BLS certification required.
• Experience with EOS model is preferred.
• Graduate of an accredited LPN program.
• Experience in a health center setting is a plus.

Responsibilities

• Oversee care for patients transitioning from facilities with follow-up calls and coordination with PCP teams.
• Coordinate care for at least 30 high-risk patients using risk stratification and registries.
• Arrange follow-up visits and manage patient data on symptoms, meds, and support.
• Handle tasks including preparing medications for clinicians and monitoring devices like ABP and CGM.
