Qualifications

Responsibilities

• Demonstrate efficient use of computer systems including data entry.
• Serve as liaison to external case managers, family, physicians, and community resources.
• Train and educate patients, families, and care teams.
• Implement standards of practice for care management and coordination of care.
• Exhibit excellent customer service and a positive attitude toward patients.
• Assist in evacuation of patients; maintain dependable attendance.
• Communicate effectively within an interdisciplinary team.
