Job summary

Optalis Healthcare's Mill Run Health & Rehabilitation Center in Ohio seeks a Licensed Practical Nurse (LPN) to provide direct resident care and ensure compliance with state and federal regulations.

Responsibilities

• Coordinates resident care with the Administrator, Director of Nursing and other disciplines to ensure quality care.
• Ensures care plans are implemented and updated based on resident needs.
• Make effective decisions to maximize clinical outcomes for residents.
• Responds to call lights and communicates needs when unable to assist directly, prioritizing requests.
• Completes daily rounds and provides shift reports with accurate, concise resident information.
• Demonstrates reliability, accountability, and effective communication and documentation.
