Job summary

St. Francis Health Services is seeking an LPN Care Coordinator in Little Falls, MN. Qualifications

Responsibilities

• Oversee day‑to‑day clinical activities in collaboration with DON and Administrator.
• Act as Nurse in Charge when delegated, ensuring regulatory compliance.
• Participate in quality improvement and care planning processes.
