Job summary

Carefree Living in Babbitt, MN, is seeking a Licensed Practical Nurse (LPN) Care Coordinator to join our dedicated assisted living team. You will work within a supportive interdisciplinary group to deliver high-quality, resident-centered care and participate in ongoing quality improvement initiatives.

You will coordinate care plans with RNs, assist with admissions, and ensure safe medication administration.

Qualifications

• Current Minnesota LPN license.
• Strong communication with residents, families and staff.
• Experience in Assisted Living preferred.
• Familiarity with Alzheimer's/dementia care is a plus.
• Comfort using electronic documentation systems.
• Team-oriented and supportive work culture.

Responsibilities

• Assist residents to reach and maintain independence through activities of daily living and scheduled services.
• Collaborate with registered nurses on care plans and care conferences.
• Assist with admissions process as directed.
• Safely administer medications and treatments per doctor orders.
• Complete documentation with accuracy.
• Mentor and support Resident Assistants; manage medication orders and refills.
• Conduct supervisory visits, chart reviews, and ensure policy compliance.
