Qualifications

• Licensed practical nurse with current license and ability to practice.
• Experience in comprehensive homecare, day services, and 245D non-residential services.
• At least 1 year of direct care experience with individuals with disabilities.
• Valid driver's license and ability to pass a motor vehicle record check.
• No disqualifying criminal background; ability to read and write in English.

Responsibilities

• Provide direct care as scheduled by the CSS, including on-call duties.
• Assist with developing assessments, outcomes, progress and health reports.
• Support CSS in implementing organizational goals and philosophies.
• Assist in orientation and supervision of new staff.
• Help with finances as assigned in CSSP/CSSPA; oversee site management.
• Coordinate medical appointments, treatments, and related paperwork.
