Life Care Center of Greeley is seeking a Care Manager Assistant - LPN to help coordinate patient care and services. The role promotes quality care through collaboration, monitoring, and evaluation of services in line with laws and Life Care standards.

Ideal candidates hold an associate's or bachelor's nursing degree and an active LPN license, with 1 year of post-acute clinical experience preferred and familiarity with discharge planning and utilization review.

Formación

• Nursing diploma (associate's or bachelor’s degree in nursing).
• Active LPN license in good standing.
• 1 year clinical experience in post-acute care preferred.
• Experience with case management, utilization review, discharge planning preferred.

Responsabilidades

• Demonstrate efficient use of computer systems for data entry and retrieval.
• Serve as liaison to external case managers, family, physicians, and resources.
• Train patients, families, and staff in care processes.
• Implement standards of practice for care management and coordination.
• Provide excellent customer service and maintain professionalism.
