Job summary

RHD and Apis Services, Inc. are seeking a Licensed Practical Nurse (LPN) to provide comprehensive nursing care for program participants in Philadelphia, PA.

The role involves documenting care, administering medications, wound care, and coordinating with physicians and families to ensure high-quality, person-centered care. The ideal candidate will be compassionate, dependable, patient, and team-oriented, with experience in behavioral health settings.

Qualifications

• Current Pennsylvania LPN license.
• CPR and First Aid certification.
• Valid driver's license.
• Act 33/34 clearances.
• FBI clearance, as required.
• Completion of all trainings required by licensure.
• Ability to complete a physical exam, TB test, and Hepatitis B titer.

Responsibilities

• Provide comprehensive oversight, coordination, and nursing care for program participants.
• Maintain nursing documentation and medication records in accordance with agency and regulatory requirements.
• Administer medications and ensure medications are taken as prescribed and documented in the electronic MAR system.
• Provide direct nursing services including wound care, tracheostomy care, diabetic counseling, and other health-related interventions.
• Assess residents' physical health and emotional support needs and respond appropriately.
• Collaborate with physicians, healthcare providers, families, and interdisciplinary team members to coordinate care.
• Educate individuals on health conditions, medications, treatment plans, and self-care strategies.
• Maintain confidentiality and communicate significant concerns to the Nurse Manager and Program Director.
