Innovive Health is seeking an experienced Licensed Practical Nurse (LPN) to join our Home Health team in Massachusetts. You will provide skilled nursing care to patients in their homes under RN supervision and an interdisciplinary team, helping manage chronic conditions and maintain independence.

This full-time role features a predictable Monday-Friday schedule, no overnights, and the chance to build lasting patient relationships while delivering compassionate care in familiar home environments.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted Massachusetts LPN license.
• Successful completion of NCLEX-PN.
• Current CPR certification.
• Strong clinical assessment, communication, and documentation skills.
• Valid driver\'s license, reliable transportation, and automobile insurance.
• Ability to meet required health screening and clearance requirements.
• Professional references demonstrating clinical competence and reliability.

Responsibilities

• Provide skilled nursing care according to individualized care plans.
• Administer medications and treatments safely and accurately.
• Monitor patient conditions and report changes to the supervising RN.
• Collaborate with physicians, case managers, caregivers, and healthcare team members.
• Educate patients and families on disease management, medications, and treatment plans.
• Maintain accurate and timely clinical documentation.
• Support patients with dignity, compassion, and respect.
• Participate in care conferences, team meetings, and ongoing training.
