Job summary

Innovive Health in Massachusetts is seeking a Licensed Practical Nurse (LPN) for Home Health on a part-time weekend schedule. You will provide skilled nursing care to patients in their homes under RN supervision and collaborate with an interdisciplinary team.

This role emphasizes flexible shifts, no overnight requirements, and opportunities to build lasting patient relationships while maintaining balance between work and personal life.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted Massachusetts LPN license.
• Successful completion of NCLEX-PN.
• Current CPR certification.
• Strong clinical assessment, communication, and documentation skills.
• Valid driver's license and reliable transportation.
• Ability to meet health screening and clearance requirements.
• Professional references demonstrating clinical competence.

Responsibilities

• Provide skilled nursing care according to individualized care plans.
• Administer medications and treatments safely and accurately.
• Monitor patient conditions and report changes to the supervising RN.
• Collaborate with physicians, case managers, caregivers, and other team members.

You'll play a vital role in helping patients manage their health conditions, maintain independence, and improve their quality of life.

What You'll Do

• Provide skilled nursing care according to individualized care plans.

Required Qualifications
