Innovive Health in Beverly, MA is seeking a Licensed Practical Nurse (LPN) for Home Health with a Monday–Friday schedule. You will provide high-quality nursing care to patients in their homes under RN supervision and collaborate with an interdisciplinary team to promote independence and improve quality of life.

The role emphasizes patient-centered care, compassionate support, and a flexible, autonomous work environment designed to reduce burnout while delivering meaningful one-on-one care.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted Massachusetts LPN license.
• Successful completion of NCLEX-PN.
• Current CPR certification.
• Strong clinical assessment, communication, and documentation skills.
• Valid driver's license, reliable transportation, and automobile insurance.
• Ability to meet required health screening and clearance requirements.
• Professional references demonstrating clinical competence and reliability.
• Two or more years of nursing experience (preferred).

Responsibilities

• Provide skilled nursing care according to individualized care plans.
• Administer medications and treatments safely and accurately.
• Monitor patient conditions and report changes to the supervising RN.
• Collaborate with physicians, case managers, caregivers, and healthcare team members.
• Educate patients and families on disease management, medications, and treatment plans.
• Maintain accurate and timely clinical documentation.
• Support patients with dignity, compassion, and respect.
• Participate in care conferences, team meetings, and ongoing training.
