Job summary

Innovive Health is seeking a Licensed Practical Nurse (LPN) for Home Health in Massachusetts. This full-time role focuses on delivering high-quality nursing care to patients in their homes under RN supervision, collaborating with an interdisciplinary team to improve patient independence and quality of life.

You will provide skilled nursing, manage medications, monitor conditions, and educate patients and families, all while documenting care and maintaining professionalism on a Monday–Friday

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted Massachusetts LPN license.
• Successful completion of NCLEX-PN.
• Current CPR certification.
• Strong clinical assessment, communication, and documentation skills.
• Valid driver’s license, reliable transportation, and automobile insurance.
• Ability to meet required health screening and clearance requirements.
• Professional references demonstrating clinical competence and reliability.
• Associate degree preferred.
• Two or more years of nursing experience preferred.
• Experience in home health, hospice, long-term care, community health, or related settings preferred.
