Job summary

Infinite Medical P.C. is seeking a Licensed Practical Nurse (LPN) to serve as a Behavioral Health Care Manager within its Collaborative Care Model program.

This in-office, non-face-to-face role supports residents in skilled nursing facilities through medical record review, screening, care planning, and team collaboration. The LPN will work with a multidisciplinary team to provide coordinated behavioral health services, document findings, and escalate safety concerns as needed, without

Qualifications

• Active LPN license and valid credentials.
• Experience with medical-record review and documentation.
• Ability to collaborate with providers and care teams.
• Strong organizational and time-management skills.

Responsibilities

• Review patient records, diagnoses, medications, behavioral health documentation, and provider notes.
• Document validated behavioral health screening tools (PHQ-9, GAD-7, MoCA, BIMS).
• Monitor patients’ behavioral health symptoms and progress toward goals.
• Develop and maintain individualized behavioral health care plans with the care team.
• Coordinate behavioral health services and follow-up recommendations.
• Prepare organized patient case summaries for psychiatric review.
• Maintain accurate patient information in the CoCM registry and tracking systems.
• Ensure compliant documentation of all care-management activities.
• Escalate urgent behavioral health or safety concerns as needed.
